RTS Weekend Covid Agreement

Please read this document carefully as it contains important information. When you sign this document, it becomes an official agreement between you and Restoring the Soul, Inc.

Regardless of vaccination status, I agree to the following by signing this waiver. Please enter your initials below each statement.

I will follow all current CDC guidelines and those effective as of the present date.*
If I experience any COVID-19 symptoms five (5) days prior to arrival, I understand my registration will be moved to the following Men’s Intensive Weekend at no additional charge to me nor will any refund be made.*

COVID-19 Symptoms may Include a fever of 100 degrees Fahrenheit or higher, shortness of breath, dry cough, sore throat, or a diminished sense of taste or smell.

If I experience any of these symptoms I will notify Restoring the Soul immediately by emailing and forfeit my attendance at the upcoming Men’s Weekend for a later Intensive Weekend date.*
If I have tested positive for COVID-19 and have no symptoms 5 days after testing, I will wear a well-fitting mask for 10 full days after testing any time I am around others during the weekend.*
I will leave the weekend if I arrive symptomatic or become symptomatic while in attendance. I understand no refunds will be given and my registration will be transferred to the next scheduled event.*
I am willingly participating in the Men’s Intensive Weekend and assume any and all risk of possible exposure to Covid-19 and potential risks associated with Covid-19 or other public health risks.*
I will share a room with 1 to 2 other participants.*

Informed Consent

My signature below indicates that I am choosing to attend the upcoming Men’s Intensive Weekend. This decision is at my own discretion, not under force or duress from any other person. I further understand that this agreement supplements Restoring the Soul, Inc. Disclosure Statement that is part of the Men’s Intensive Weekend

I HAVE READ THE ABOVE WAIVER AND RELEASE AND UNDERSTAND THAT I GIVE UP SUBSTANTIAL RIGHTS FOR MYSELF BY SIGNING IT, AND KNOWING THIS, SIGN IT VOLUNTARILY. I AGREE TO IN-PERSON, FACE-TO-FACE GATHERING, KNOWING THE RISKS AND CONDITIONS INVOLVED AND DO SO ENTIRELY OF MY OWN FREE WILL. I UNDERSTAND THAT I AM WAIVING CERTAIN LEGAL RIGHTS THAT I MAY HAVE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE.
